Gateway Post-Acute Care at Oak Park Nursing and Rehabilitation provides skilled nursing and medically complex care to those who are recovering from surgery – such as a hip or knee replacement – or a medical event, such as a stroke or heart attack. Our goal is to get you home as quickly – and safely – as possible.

We utilize multiple modalities to aid in healing and recovery, such as:

  • Intensive PT, OT & ST
  • Complex pulmonary capabilities (New trachs, chest tubes, pigtails, and high-flow oxygen therapy)
  • Total Parenteral Nutrition (TPN)
  • Extensive wound care, including vacuum assisted closures (VACs)
  • IV Therapy (PICC and Central lines)
  • Specialized menus to enhance healing and stabilization
  • Aftercare education for symptom management
  • In-home therapy evaluation prior to discharge
  • Outpatient therapy services

In addition to excellent medical care, you’ll enjoy wonderful amenities which help aid in healing. These include:

  • 18 private medical suites, each beautifully appointed with a television, telephone, and private bathroom
  • The Gateway Café, a private dining area with a private patio for outside recreation and meals
  • Designated private rehabilitation area for the Gateway patients

Our Gateway team consists of people committed to your success, all of whom have specialized training in meeting the needs of post-acute care patients.

  • All nurses are certified in Basic Life Support services and trained to provide complex medical care
  • Our Rapid Response Team is available 24/7 in case of emergency
  • Physical Medicine and Rehabilitations (PM&R) specialist
  • Wound care nurses
  • TPN qualified staff members for TPN administration
  • Registered dietitian

Commonly treated conditions include:

  • Surgical aftercare (CABG, ORIF, surgical drains, chest tubes)
  • COPD
  • Simple and complex pneumonias
  • Cardiomegaly/ Cardio myopia, and CHF
  • Infections requiring IV antibiotics (Peripheral, central and PICC lines)
  • Other medically complex conditions requiring multi-disciplinary medical staff and specialist
